Lasting Power of Attorney (LPA)
Purpose: Appoint someone you trust to make decisions on your behalf in the event that you lose the mental capacity to do so.
How it makes your wishes known: Provide clarity on whom you trust to make decisions on your welfare, property and finances when you are not able to speak for yourself.
What to do:

Step 1 - Consider whom you wish to appoint to make decisions on your behalf (called Donees) and the powers to be granted to them.

Step 2 - Complete the online LPA form (10 minutes).

Step 3 - Your Donee(s) will receive SMS notifications to accept the appointment(s).

Step 4 - After your Donee(s) have accepted the appointment(s), schedule an appointment with a Certificate Issuer to certify your LPA.

Step 5 - Meet your Certificate Issuer, who will ensure you understand the purpose of the LPA and the scope of authority you have conferred on your trusted persons.

Advance Care Planning (ACP)
Purpose: Record your healthcare preferences for potential end-of-life or mental incapacity situations.
How it makes your wishes known: Make your medical preferences explicit, so that your doctors and those you trust can tailor your care and treatment in a way that respects your preferences, when you are not able to speak for yourself.
What to do:

Step 1 - Create your Advance Care Plan (30 minutes).

Step 2 - Schedule an appointment with an ACP facilitator to register your plan.

Purpose: Specify how you'd like your CPF savings to be distributed when you pass on.
